These professional services are billed separately from any facility/office charges or professional services billed by your surgeon, dentist or other specialists who may be involved in your care.
For In-Office Dental Anesthesia Services
For patients with private dental insurance who have had anesthesia services provided at their dental office, Collaborative Anesthesia Partners will provide a ‘Super Bill’ itemizing your professional anesthesia services to your email on file. This ‘Super Bill’ can be used by you when seeking reimbursement from your insurance provider. Collaborative Anesthesia Partners does not seek private third-party reimbursement for anesthesia services rendered in the dental office setting.
